- South African Citizen: A legal citizen of South Africa, including South African nationals and
those with a South African ID
- Permanent Resident: Those who have gone through the immigration process and approved for permanent residence
- Asylum Seeker: Those who have fled their country and are seeking protection as a
refugee but have not yet received refugee status
- Refugee: Asylum seekers who have received refugee status
- Immigrant: Those who have moved to South Africa seeking permanent residence
but not yet been approved
- Migrant Worker: Those who moved to South Africa for labor work

If ‘outdoor/street address’ is selected as the ‘where client slept last night’, a new field will appear. Indicate the closest approximate location if the client was sleeping outdoors or on the streets last night. |
Reason homeless |
- Addiction: The PRIMARY reason that the client is without a home is a result of the client’s
struggles with alcohol and/or drug addiction
- Economic: The PRIMARY reason that the client is without a home is due to economic difficulties including job loss, struggle to pay for shelter, loss of source of income, etc.
- Family Conflict: The PRIMARY reason that the client is without a home is due to family conflict including divorce, separation, being kicked out, etc.
- Trauma: The PRIMARY reason that the client is without a home is the result of a traumatic
experience including domestic violence, abuse, assault, loss, threat, etc.
- Mental Health: The PRIMARY reason that the client is without a home is due mental health
instability, which can result in the inability to get a job or stay with their family
- Other: The PRIMARY reason that the client is without a home is not listed above

- Low: Should be selected if there is little to no vulnerability or concern for the client’s safety within the shelter (social worker’s discretion)
- Medium: Should be selected if there is some concern for the client’s safety within the shelter and they express some vulnerability (social worker’s discretion)
- High: Should be selected if there is concern for the client’s safety within the shelter and
they have expressed vulnerability (social worker’s discretion)

- Low: Should be selected if there is little to no risk for other clients or staff members from the new client (social worker’s discretion)

PDP DATE COMPLETED |
Indicate the date the client completed their PDP. This should only be entered in if the client has met with their social worker and completed their PDP. This is this date that all of the PDP information is finalized |
Planned exit date |
Indicate the planned exit date of the client as indicated on the PDP. This should only be indicated if the client has met with their social worker and completed their PDP. (This is a required field)
This is the date that is indicated on the PDP for when the client should be planning to leave the Haven after completing the other necessary steps in their PDP, not just 6 months out from the entry date. |
Reason Exit |
- Absconded (PDP): A client left on their own accord, without notification, post-PDP completion
- Absconded (No PDP): A guest left on their own accord, without notification, pre-PDP completion
- Reintegrated: A client was successfully reintegrated into society. This could mean they have found gainful employment and/or acquired new living quarters.
- Reunified: A client was successfully reunified with their family which could mean they moved back in or have established communication with their once estranged family.
- Transfer: The client is being transferred to another shelter
- Termination of Service: The client was unruly or combative and the Haven decided to terminate services.
- Overnight Stay: Guest stayed a night or two and was discharged after this stay as they decided not to partake in Haven’s services, but notified the Haven of their departure.
- Deceased: The guest or client died while staying at the Haven.
